This post is going to be somewhat long because I’m looking for advice and help regarding my financial situation. I’ll be as transparent as I can be and I would really appreciate any help or advice in what to do in my situation because right now it just feels hopeless.
Where to begin…so the financial situation that I’m in right now is one that feels like I don’t have any way out of, and while some of it is absolutely my fault, a lot of it is circumstances just not working in my favor and life sometimes just giving you a swift kick while you’re down. I’ll start with some back story. So I came out of 2020 in a not-so-terrible financial situation. I took out a large high interest loan (due to inexperience in that world) and was right near paying it off when 2020 hit. I buckled down, adjusted my finances and while I struggled I was able to pay that loan off entirely and get my finances mostly in check despite my job being shut down for over a year and a half and surviving on what I had saved. While it was frustrating watching years worth of savings dwindle away, I was both happy and proud of myself that I was able to survive and make it work. I had to move out of my place, sell most of my belongings and move in with a friend for a time but I made it work.
At the beginning of 2022, around March, I started a new job in a different field that pays well and much better for my mental health and overall work/life balance. I make decent money, I don’t pay outrageous rent and I’m not spend happy or frivolously wasting my money on unnecessary things. I found out that I had a sizable amount of debt still from credit cards that my then partner was using in my name without my knowledge. When we split up around the end of 2021 she moved back to her home country and basically left me with almost $30k in debt that I had no knowledge of until she was gone. I did the only thing I knew to do, and in July of last year, I took out another personal loan. This one was a bit more urgent given my financial situation so it may not have been the best out there, but it took care of the bulk of my credit and I was able to get to a sustainable place where I could at least see the light at the end of the tunnel. The new job required that I purchase a new car, and my company has a work program that helped me purchase it, but it was another loan taken out in my name. I didn’t buy anything crazy expensive or fancy, I wanted nice and reliable but also affordable so I bought a Honda Civic. I love this car, it’s mine, I’m proud of it and it allows me to do my job which is sales based so I’m on the road for the most part every day traveling around my city to different accounts.
Then in the middle of 2022 my grandmother, who is basically like my mother, went from staying with my aunt to needing to move into a hospice. My family does not have much money and I am on the other side of the country from them so I made arrangements with the hospice facility to shoulder most of the bills related to the home and my grandmother’s needs without my family knowing. As far as they know, a lot of the money that pays for my grandmother is donation based from her church and I would prefer that it stay that way. My family is very religious, conservative and prideful so if they found out that I was the one paying for this sadly it would do much more harm than it would do good to our relationship. We’ve already had several fights about the needs of my grandma and who would pay for them, how much they would pay and that type of uncomfortable conversation surrounding a dying loved one. Well, sadly my grandmother passed away peacefully 2 weeks ago. She donated her body to a local university and we didn’t have a funeral, cremation or anything of the sort. I flew home and thankfully through a friend of mine was able to reserve a local American Legion Hall for free and we had a lovely ceremony remembering a beautiful soul that I’ll miss forever.
Now this lands us to today. I am currently around $50k in debt, with $25k of it being in a personal loan I took out and the rest being split up between a few different credit cards. This does not include my auto loan, but that payment is very manageable and I see no issue with allowing that to stay where it is. What I’m trying to accomplish right now, is finding a loan that would be $50k that I could consolidate everything I have into one payment that in a perfect world I could keep around $1000 per month. I don’t even know if this is possible, but as my credit to debt ratio has gone up my credit score went from a 750 to now somewhere around a 670. I have reached out to several different loan companies trying to get a loan in this amount, but given that I just took out a personal loan last year AND an auto loan, I’m not approved for anywhere near that amount. This is the dilemma.
The issue I’m having right now is that the loan payment is $900 per month, plus all the other credit cards I’m paying entirely too much and slowly digging a deeper hole, which forces me to put some necessary expenditures on a credit card, raising my debt to credit ratio, which then lowers my credit score and it just feels like the hole is slowly getting deeper and deeper. I’m not living beyond my means, I’m not being silly or irresponsible with my money. Life handed me a shit hand and I did the best I could with the tools I had at the time to survive this and now I just don’t see a way out. I’m at my wits end trying to find a way to make this work and it’s frustrating because I know I can afford to pay this monthly amount and if I can consolidate everything into one loan I could pay those credit cards off which would bring my credit score back up and I’ll be back in a place where with 4-5 years of paying off that loan I’ll be back at square one and out of debt entirely and able to begin saving again and getting back to where I was financially before the pandemic.
I would really appreciate if anyone has any experience with a situation like this and can point me in a direction of what could be helpful. I get letters all the time about a “debt forgiveness program up to $50k” but I’m fairly certain all that’s going to do is default on all of my debt and basically destroy my credit score to oblivion and be on my record forever having done that so I’m really trying to avoid that or declaring bankruptcy. I simply don’t know what to do and I need help.

Background info: I live in Portland, Oregon, and I'm covered by SAIF insurance. On Sunday I took a fall down a few stairs and grabbed the handrail with my left hand to catch myself; my left shoulder took the brunt of the force. I was clocked in and I have a witness to the injury.
Since then my left shoulder has been stiff/tender and I have lost some range of motion, especially having to rotate it to put on shirts, and it just sounds crunchy and poppy. I'm going today to an urgent care with my claim form to get it checked and seek a referral for more investigation but was wondering if it would be more beneficial just to seek a specialist in the matter? I worked with a medical group (Rebound Orthopedics) who takes workers comp, but I know claims are complicated and one poor decision can run the risk of less being covered.
My worry is that the urgent care won't take it seriously (I had a frustrating experience dealing with poor treatment and misdiagnosis on a blown knee from a sports injury last year) and I want to make I'm prepared to navigate this process since this is my first time dealing with workers comp so any advice would be greatly appreciated!

My girlfriend has been in tremendous amounts of pain for nearly 2 weeks now, we've gone through the ER and urgent care separately and both of them gave us the "We couldn't find anything here's a huge bill" treatment, so there's no chance we can afford to go back to the ER for it and it's not getting better. I'm unsure of what to do about it at this point, so I'm hoping someone may have some advice.

Hello
I've had pain in knee for about 8 weeks. Yesterday I went to a walk in sport ortho urgent care. Diagnosed with medial torn meniscus. I received prescription for oral steroids and some NSAIDs. Also signed up for 2 weeks of pt before the next check up.
So much conflicting information on the internet. Certain tears can heal others can't, age matters(about to be 32) red zone, white zone. Never get surgery, PT is a waste. So I'm not sure what to do.
Should I get an MRI done to see how bad it really is?
Should I get a 2nd opinion from an established orthopedic dr vs walk in urgent care?
I just don't want to waste time and money on PT if I the tear is in a bad no heal zone and have to get surgery any way and then PT afterwards
Please advise.

26F. In mid 2021 I started noticing blood when I wiped. I had always dealt with "stomach issues" and passed it off as IBS. But when I noticed the blood, I reached out to my GP about it. She prescribed me some cream for hemorrhoids.
In November 2021, I got what I thought was food poisoning. I was unable to eat for 2 days and was going to the bathroom nonstop. After that, I was never the same. I got my appetite back but the diarrhea was always there with blood dripping almost every bm. It started becoming really painful to use the bathroom and after googling, I thought I had a fissure. I continued using the cream my GP gave me, along with wipes and Lidocaine for the pain.
I managed. Then in July 2022, the blood and diarrhea was bothering me a lot. I set up an in person appointment with GP. Again, she said it was hemorrhoids and suggested I take a fiber supplement.
In early November 2022, I get 2 more bouts of "food poisoning." Shortly after, I went to urgent care with terrible rectum pain. I couldn't move without feeling pain. The doctor said it was hemorrhoids but at this point I was through with no one acknowledging that I had diarrhea for years. He referred me to GI and general surgeon. General surgeon reached out to me first and when I saw her, she stuck her finger up me and said you have a fissure. She gave me topical ointment. GI called me asked me a few questions and told me to keep a food journal and he'd follow up with me after the holidays... I wasn't satisfied and I made an appointment with my GP (dumb decision, she obviously was doing nothing for me). I asked for stool tests. She runs a few for parasites and bacteria and I come back positive with H pylori. She gives me antibiotics. I finish them mid-January.
Late January 2023, I started seeing mucus and blood clots in my diarrhea. I called the GI office and asked for another phone appointment. February comes around and I started getting painful bumps on my legs. Google search shows erythema nodosum and how it's associated with IBD. I'm in so much pain I can't stand, my legs/feet are swollen and bruised. It's awful.
I go to GP about the bumps, she says it's erythema nodosum and gives me meloxicam. It doesn't improve and actually gets worse, so I go to a derm and he does a punch biopsy.
While I'm waiting for the results of the biopsy. GI finally calls me back. At this point, I lost around 10 pounds from my "food poisoning." He schedules me for a colonoscopy in April. Asks me to submit blood and stool tests. I tell him about my erythema nodosum and he says he'll be in contact with my derm.
Submitted the blood and stool tests. All inflammatory markers are elevated and cal protectin >800. GI calls me back and says he wants to get me in for a colonoscopy earlier than April.
Had my colonoscopy on March 23. Says I have mild colitis of the entire colon but he says terminal ileum is normal. Took biopsies and says he call me back with results.
4 days ago, GI calls me. Says he wants to hold off medicine so he can do an MRI to rule out Crohn's because biopsy result wording is not clear???? I agree (because wtf else am I supposed to do) Then few minutes later he calls me back and says nvm I want to start you on treatment due to the erythema nodosum. It still has not cleared and I have developed new lesions (though the swelling and pain has def gone down).
And now here I am. 4.2g of mesalamine (4 pills) taken this morning with my breakfast. Still coming to terms with the idea of taking medication my entire life and really hoping not to have to do more invasive treatments like biologics... It's been a long hard journey and I know i have more to go.

Over the past month or so, my SO and I have really taken our health seriously and started eating a super clean diet, cut out soda and energy drinks, added light exercise, and have overall just started feeling better.
Yesterday, I was watching my daughter's dance class and I started to feel super dizzy, light headed, heart racing, etc. I had my friend take my daughter to her house, went to urgent care, and called my MIL to go pick up daughter from friends house.
Urgent care didn't really find anything other than my blood sugar level was 79, but they said that was within range. My SO said maybe I should start adding more carbs and sugars back.
Cue my MIL saying she thinks I'm skipping meals because she doesn't see me eat at all any more, and she's used to me "gobbling and gobbling food down all day, eating all the time, never not eating something".
I'm not skipping meals.
She really brings up me eating food any chance she gets. Along with a myriad of other comments about parenting, work, life, everything in general, about how I'm doing something wrong, her way is right. And it's usually ends with comments about how other people tell her they just looooooooooove her.

My psychiatrist retired a couple months back, I’m having trouble contacting him, and I still have yet to find a new psychiatrist that I trust well enough to meet with. My prescription is about to run out in a week, and if I run out I will have seizures. Does anyone have any advice? Can I go to urgent care and get a script for another month till I figure stuff out? 2023.03.30 17:20 swhang77 Repair shop by Insurance (Geico) or Tesla Repair Shop for a hit and run incident
My M3 was charging and then I got a sentry notification on my phone. I went and checked it and a car had backed up into my car and then fled the scene. Filed a police report and they said they couldn't file any charges since the person in the car wasn't identifiable. Reported it to my insurance and they looked up the license plate # and contacted the other insurer, which is Geico. I handed over the video and they IMMEDIATELY took responsibility. So glad for sentry mode!!!
Geico said they can take care of anything, all I need to do is drop it off at one of their repair shops in their network and that it'll have priority so that it'll get fixed quickly. Plus, all the rental car stuff will be handled too and so it makes it super seamless.
However, they also said I can take it to any repair shop and Geico will just sent me a check of what their estimate is. I have heard Tesla Repair shops could take forever and it could exceed the length of the rental car stipend that Geico provides, which they said they won't give me details b/c the damages and car rentals are in a single coverage limit from their end.
Any recommendations if I should go with Geico's network or go with the Tesla repair shop? No need to be urgent for me as it's just front bumper damage.

Hello!
(Note: So, I'm fairly new to Reddit so bare with my lack of experience in this setting. :>)
I have been a PA-C almost 5 years, and took a job working in geriatrics right out of school. I mainly work in SNF's and some limited time in assisted living arena, but no hospital medicine. I also work a limited amount PRN in Urgent care. I struggled with the immediate independence early on, as I rarely see my SP or even talk with her. She is "available" but only to a limited degree, so I've done a lot of my own learning about the SNF systems and how they operate. Early on struggled with juggling time, since we have to travel between facilities, place our own schedules in EMR, dictate, bill, etc. on our own. So at this point, I've gotten this all pretty much down, although I still struggle with accruing general medical knowledge and true CME due to schedule conflicts and family time, basically doing my best to listen to a podcast 2-3x a week in the car.
So my conundrum is that I am getting a bit tired of geri (i.e. schedule, pt loads, lack of skills/hands on) and wanting to (possibly) learn a new knowledge set and/or get into a specialty where I see I'm making a bigger impact on pts and have a tighter knowledge base. Maybe some hands on skills required.
Not enough time to go into great length about all the details, but suffice to say I don't get a lot of SP time/support, schedule is a bit unpredictable, and feel need to find a niche. My current schedule is M-F varying 8-5 or some days could be 11-7 if charting needs done or coming off call (6-7 nights a month, phone only, moderate volume calls). Pay is above average, but not great (103k, sucky call hr call pay, RVU bonuses pretty standard).
For those who have chosen to jump ship into a new specialty (with a family depending on you) how did you do it and what specialty did you break into? Also, what did you do prior to jumping ship? Really summing it up, I'm afraid to make a move and get hired into another role that's unfamiliar and end up loathing it or getting fired for lack of knowledge/experience. If I was single without kids, it wouldn't be such a hard sell.
Any advice from the PA-C community much appreciated. Thanks for hearing out my ramble.

Buckle up..
So I have known this guy for a while now, we used to be really good friends. I should've known to not get involved with him considering he has cheated on every one of his past girlfriends. Anyways, I had a an STD test done earlier with my pap smear and everything was negative. I hook up with this guy and 4 days later I have what I thought was the worst yeast infection. I treated it at home. Something was telling me it wasn't right as I had the worst pelvic pains so 2 weeks later I went to an urgent care and got a test done. Came back as + for chlamydia.
I reach out to him and tell him in a very nice way...and it immediately backfires on me. He claims he hasn't slept with anyone, he's not believing me until he sees my "full medical history with my name and time stamp" (basically saying that I'm lying to him about this), that my story doesn't line up..etc. I sent him the voicemail of the "call back for your results" and he claimed I was lying about it. I start getting upset and send him my past results (before him) showing I was clean before him (like full nucleic acid test not urine). He starts blaming me saying I had sex with someone before him and he refuses to get tested. He is from a very prominent family and now is threatening to blackmail me or take legal action. He won't stop contacting me accusing me of giving it to him...kinda at a loss of what to do. He said to get back at me he is going to "send videos to my ex of 6 years". I am afraid to block him. Also, unbeknownst to me until this situation came up, that there is another female that I did not know about who he refuses to tell "who he loves and I put him in this situation".
I learned my lesson (so spare me the wrap it up jokes cause trust me..) but any advice on how to solve this would be great. Unless I had sex with a chlamydia ghost, idk what is going on and how to solve this. At this point, I am about to take a lie detector test lmao.

My fiancé has been dealing with swelling in his legs since about early/mid January. We went to the urgent care at first where they did an EKG and didn’t see much but they sent us to the ER worried about a blood clot. The ER sort of brushed him off thinking he was young and relatively healthy. His d-dimer there was just below positive at 0.48. He’s been having a bunch of testing done since. Since then he had another d-dimer done and it was 2.5 and his doctors didn’t do anything about it. One of his CT scans showed enlarged retroperitoneal lymph nodes which prompted them to do a testicular ultrasound which showed nothing. He had problems with some valves in his legs which partially explained the swelling. They also found a nodule on his right lung that they weren’t too concerned about, as well as evidence of a reactive lung disease but he has had asthma his whole life. On his most recent echo though, they found trace regurgitation in his mitral, tricuspid, and aortic valves as well as a pulmonary artery systolic pressure of 34 mmhg which they wrote in the results as mild pulmonary hypertension. His current primary care has not been doing well on getting on top of any of his results. Does anyone have any suggestions on how to go about dealing with this? I know an echo alone can’t diagnose this, but I want to get on top of it as fast as possible if this is what he has.
